Senior Medical Questions

Has any proposed insured ever been rejected for insurance by American Income Life?
Has any proposed insured been advised to have or had a heart, lung, liver or bone marrow transplant?
Has any proposed insured been diagnosed or treated for cirrhosis, Alzheimer's disease, ALS or senile dementia?
Has any proposed insured had a toe, foot or leg amputated due to illness or disease?
In the past 2 years, has the proposed Insured been hospitalized for heart attack or stroke or been diagnosed or received treatment for cancer (other than non-melanoma skin cancer) or other malignant tumor or growth?
Is the proposed Insured bedridden, hospitalized, a resident of a nursing facility or require oxygen equipment to assist in breathing in the home setting?

Medical Questions

Has any proposed insured ever been convicted of a DWI or DUI?
Has any applicant ever been arrested?
Has any proposed Insured ever had or been treated for Angioplasty, Coronary Bypass, Heart Attack, Heart Failure, Angina, or Artery Disease?
Has any proposed Insured ever been diagnosed as having or received treatment for high blood pressure?
Has any proposed Insured ever had or been treated for Diabetes?
Has any proposed Insured taken medications for Anxiety or Depression?
Has any proposed Insured ever been diagnosed as having or received treatment for cancer, tumor or unexplained masses (including benign tumors)?
Are you currently taking or have you been prescribed medications by a doctor in the past five years?
In the past 5 years, has any proposed insured been to the ER or Hospital?
Has any proposed insured ever been diagnosed with or treated for Crohn's, Cystic Fibrosis, COPD, Seizures, Scleroderma, Alzheimer's, Multiple Sclerosis (MS), Stroke, Sickle Cell Anemia, liver, or kidney issues?

What Comes Next?

Underwriting Process

  1. The client knows a prescription history check will verify all prescription medications they've had over the last ten years and will be compared to what is listed on the application.
  2. Medical records may be ordered from their physicians.
  3. An exam may be requested which will be done privately in their home by a licensed nurse wearing full protective equipment.
